    ⚠️  Witness Requirement: All witnesses must be unbiased adults who are NOT beneficiaries of the estate. Witnesses are the client’s responsibility to arrange. ARTSA will coordinate and verify eligibility — but does not provide witnesses by default. Qualified witnesses can be supplied for $35 per witness with advance notice — not assumed on arrival.

Multiple Locations Scope Limit: Limited to locations within 20 miles of each other, same day. Greater distances or multiple days require a separate custom quote.

Do You Have What You Need?

Core documents — everyone needs these four:

  • Last Will and Testament — names guardians, distributes assets, names executor
  • Durable Power of Attorney — names someone to handle finances during incapacity
  • Healthcare Power of Attorney — names someone to make medical decisions for you
  • Living Will / Healthcare Directive — states end-of-life care wishes

Additional documents — depending on your situation:

  • Revocable Living Trust — avoids probate, manages assets during incapacity
  • Pour-Over Will — catches assets not transferred to trust
  • Trust Certification / Abstract — for banks and title companies
  • HIPAA Authorization — allows named people to access medical records

⚠️  If any of your documents are not yet Signed and Notarized — your plan is incomplete. Every day you wait is another day your family is unprotected.

FAQ - ANSWERS TO COMMON QUESTIONS

Q: Can you help me fill out my DIY forms?

No. ARTSA cannot provide legal advice or help complete documents. ARTSA only executes documents you have already prepared

ARTSA will point out obvious errors (missing dates, unsigned sections) but cannot advise on whether your planning choices are correct. If concerning issues are found, attorney review will be recommended

No. Everything is transparent. Professional tier: $275 including travel up to 40 miles. Recording fees are separate. Additional witnesses $35 each with advance notice.

Cash, check, Venmo, Zelle, credit/debit cards. Payment due at time of service, with a pre-paid retainer equal to time and travel expenses AND paid in full prior to any notarization services. 

Need to reschedule? Generally, there’s no problem. No fee with 24-hour notice. Retainer fee under 24 hours. Full fee for no-show.
