1. Please scroll down until the service cart selection appears.

  2. Enter the total number of Principal Signers within your document(s), not matter how many times they will sign.  Add any credible witnesses (see below) or signature witnesses, when required to complete the act.

  3. This combined number of Signers is the total amount for your service order. Please indicate that number in the cart selection at the top of this page.

Under Tennessee Code Annotated § 66‑22‑107, a credible witness may be used only if:

  • The signer lacks acceptable forms of identification. (This does not include in cases of causing an inconvenience for the principal signer)

  • The witness personally knows both the signer and the notary public is personally acquainted with the credible witness.

  • The notary can rely on the witness’s affirmation of the signer’s identity under penalty of perjury

If this is a Jurat, or Affidavit, complete any possible blank spaces, but DO NOT sign the document until in the presence of the notary, after ID verification has occurred.

If this is an Acknowledgement, your document to be notarized may have already been previously signed before being in the presence of the notary. The acknowledgement itself will not yet be signed until in the presence of the notary, and after ID verification has occurred. You will be attesting to the fact that you yourself previously signed the document, were aware of what you were signing, and understand its contents.
